1. Phacoemulsification — The Gold Standard

Phacoemulsification (commonly called “phaco”) is the world-standard cataract surgery and accounts for over 95% of my cataract cases. A 2.2 mm incision (no stitch) is made at the edge of the cornea, a circular opening is made in the lens capsule (capsulorhexis), and a tiny ultrasound probe emulsifies the cloudy lens into fragments that are gently suctioned out. A folded intraocular lens (IOL) is then injected through the same incision and unfolds inside the eye. 2. Femtosecond Laser Cataract Surgery (FLACS) — Bladeless Precision

For patients who want the absolute pinnacle of precision, I offer bladeless femtosecond laser cataract surgery. The LenSx femtosecond laser performs the corneal incisions, the capsulorhexis (perfectly circular and centred) and fragments the lens — all without any blade or ultrasound energy in the early stages. 3. Manual Small-Incision Cataract Surgery (SICS)

For very dense, mature or hypermature cataracts where phaco may be unsafe, or for patients who prefer a more affordable option, I perform manual SICS — a self-sealing 6-7 mm scleral tunnel incision through which the entire lens nucleus is removed and a rigid IOL is implanted. 4. Pediatric Cataract Surgery — For Newborns & Children

Cataract surgery in a child is completely different from adult cataract surgery — the lens is soft, the eye is still growing, and the brain’s visual system is developing. I perform:

  • Lensectomy-vitrectomy — for newborns with dense congenital cataract, the lens and anterior vitreous are removed through a pars plana approach.
  • Phaco-aspiration with primary posterior capsulorhexis & anterior vitrectomy — for older infants and children, to prevent PCO.
  • IOL implantation — delayed in infants (after 2 years) but primary in older children, with biometry adjusted for the growing eye.

5. Traumatic Cataract Surgery

Cricket ball injury, road traffic accident, firework injury, or sharp trauma — all can cause cataract with associated zonular weakness, iris damage, or lens capsule rupture. I perform staged traumatic cataract reconstruction with capsular tension rings (CTR), iris repair and scleral-fixated IOLs where needed. PubMed evidence on traumatic cataract.

6. Secondary IOL Implantation

Patients who are aphakic (no IOL — often from old surgery or trauma) need a secondary IOL. I implant:

  • Scleral-fixated IOL (SFIOL) — IOL fixed to the sclera with polypropylene haptics. Best for eyes without capsule support.
  • Iris-fixated IOL (Artisan/Verisyse) — clipped to the iris. Excellent visual outcomes.
  • Anterior chamber IOL (ACIOL) — older technique, used selectively.
  • Posterior chamber IOL in the bag / sulcus — when capsule support exists.

7. Combined Cataract & Glaucoma Surgery

For patients who have both cataract and glaucoma, I perform combined phacoemulsification with trabeculectomy or phaco with iStent inject (MIGS — Minimally Invasive Glaucoma Surgery). One surgery, two problems solved.

8. Combined Cataract & Vitrectomy Surgery

For diabetic patients who have both cataract and vitreous haemorrhage / retinal detachment, I perform combined phaco + pars plana vitrectomy — saving you a second surgery.

9. IOL Exchange & Repositioning

If your previous cataract surgery (done elsewhere) left you with the wrong IOL power, a decentred IOL, or a subluxated IOL, I perform IOL exchange or repositioning with scleral fixation.

10. YAG Laser Posterior Capsulotomy

The “secondary cataract” or posterior capsule opacity (PCO) is a quick, painless, in-office YAG laser procedure. It takes 2-3 minutes per eye, requires no anaesthesia, and vision is restored within hours. PubMed evidence on YAG capsulotomy.

Premium Intraocular Lens (IOL) Types — Which Is Right for You?

Choosing the right IOL is the single most important decision in cataract surgery after choosing your surgeon. The IOL is the artificial lens that replaces your cloudy natural lens, and it will stay inside your eye for the rest of your life. At Crystal Clear Eye I offer the complete range of IOLs from the world’s top manufacturers — Alcon, Zeiss, Johnson & Johnson (AMO) and Bausch & Lomb.

1. Monofocal IOL (Standard)

The most commonly implanted IOL in India. Provides sharp distance vision, but you will need reading glasses for near. Best for patients who don’t mind wearing glasses and have no significant astigmatism. Most affordable. Examples: Alcon AcrySof IQ, J&J Tecnis 1, Zeiss Asphina.

2. Multifocal IOL

Multiple focus zones built into the IOL provide sharp vision at near, intermediate and distance — most patients are completely spectacle-free. Ideal for active people who hate glasses. 3. EDOF IOL (Extended Depth of Focus)

The newest generation of premium IOLs. Provides a smooth, continuous range of vision from distance to intermediate (computer / mobile distance) with minimal halos. Excellent for office workers, drivers and phone users. Examples: J&J Tecnis Symfony, Alcon Vivity.

4. Toric IOL

If you have astigmatism (rugby-ball-shaped cornea), a toric IOL corrects both your cataract and your astigmatism in one surgery. Available as monofocal, multifocal or EDOF toric. Without a toric IOL, you will need glasses after surgery even if the surgery is perfect. PubMed evidence on toric IOLs.

5. Accommodative IOL

Designed to flex inside the eye to give some near vision. Less popular now that multifocal and EDOF IOLs have improved.

6. Light-Adjustable IOL (LAL)

A futuristic IOL whose power can be fine-tuned with UV light after surgery — the only IOL that lets us “test drive” your vision before locking it in. Available at select centres.

7. Scleral-Fixated / Iris-Fixated IOL

For complex cases where the natural lens capsule is damaged or absent (trauma, prior surgery, subluxation). The IOL is fixed to the sclera or iris using special sutures or haptics.

How I choose the right IOL for you: I do a 45-minute IOL workup that includes IOLMaster biometry, Pentacam corneal topography, OCT macula scan, specular microscopy and a lifestyle questionnaire (do you drive at night? use computers all day? hate glasses?). Based on this, I recommend 2-3 IOL options — you choose.

From Newborns to 100-Year-Old Patients — Cataract Surgery Knows No Age Limit

One of the things I am most proud of at Crystal Clear Eye is the sheer age range of cataract patients I have operated on. Most eye hospitals in Mumbai will tell you “we treat all ages” — but very few actually do. Below is the real story of the age spectrum at my clinic.

The Youngest Patient — A Newborn with Congenital Cataract

Congenital cataract affects approximately 1-3 in 10,000 newborns in India. If left untreated in the first 6-10 weeks of life, the baby’s visual cortex will never develop normal vision — a condition called deprivation amblyopia. I have operated on babies as young as 4 weeks old, performing lens aspiration with primary posterior capsulorhexis and anterior vitrectomy, often with delayed IOL implantation. Children & Teenagers (1–18 Years)

Traumatic cataract (cricket ball, fall, firework), developmental cataract, steroid-induced cataract (in young asthma / juvenile arthritis patients) and post-uveitis cataract are routinely managed. Pediatric cases require specialised anaesthesia and lifetime amblyopia monitoring.

Young Adults (18–40 Years)

Traumatic cataract, post-LASIK cataract, steroid-induced cataract and early-onset diabetic cataract make up this group. IOL calculations in this age group must account for potential future refractive change.

Middle-Aged Adults (40–65 Years)

This is the peak cataract demographic. Posterior subcapsular cataract (PSC) is common in diabetics and steroid users — causing disabling glare while driving at night. Nuclear sclerotic cataract causes gradual blurring and “yellowing” of vision. This is also the age group that benefits most from premium IOLs.

Seniors (65–85 Years)

The bulk of my cataract practice. Mature white cataracts, hypermature cataracts, cataracts with pseudoexfoliation, cataracts with small pupils — all routine at Crystal Clear Eye. With modern topical anaesthesia, even patients on blood thinners can be operated safely.

The Oldest Patient — A 100-Year-Old with a Mature White Cataract

Yes, I have operated on a 100-year-old patient. With proper anaesthesia clearance, topical anaesthesia and modern phaco, age is no longer a contraindication for cataract surgery.
